Ayurvedic management of female Pelvic organ prolapse: a case series

Abstract
Pelvic organ prolapse (POP) is a condition affecting millions of women worldwide. Despite the high prevalence, it has not received sufficient medical attention. The lifetime risk of women for POP surgery is estimated to be 19 % and the reoperation risk even with an appropriate surgery is about 30%. In aayurveda, yoneeroga like prasramsinee, andinee, phalinee, mahaayoni, etc. are explained in which the features of POP in different stages can be found. A case series was prepared with 10 cases of early stages of pelvic organ prolapse and with varying associated discomforts which were seen affecting their day to day activities. The staging of the prolapse and the severity of the associated symptoms were assessed before and after the treatment using POP-Q and POP-SS tools respectively. Sthaanika ayurvedic treatment procedures yoneepicu with satapushpa coorna kalka and maatraavasti with tilataila were selected with the aim of restoring the tone of the supportive structures of pelvis thereby reducing the prolapse and associated symptoms. Before and after treatment showed a change in the POP-SS score from 8.8±3.1 to 4.5±3.4 showing a significant level of p-value 0.005. The ICIQ UI score showed a significant change from 9±2.5 to 5±2.5 with a p-value 0.005. Though the treatment was not effective in reducing the stage of POP, it was effective in managing the symptoms associated with the prolapse so that the need for surgery may be either avoided or prolonged as per the stage of the disease.
